The UIF declaration and certificate of service
Two separate obligations to the Department of Labour, and E·BIZI Pay handles them in two different places. The monthly declaration is a step on the Processing dashboard. The certificate of service is on the employee you removed.

Submit the monthly declaration
On the Processing dashboard, pick the month and choose Mark complete on Submit UIF Declaration.
E·BIZI Pay builds the declaration from the confirmed payroll and sends it. This is one of the few cards that transmits something rather than recording that you did, so give it a moment rather than clicking straight on.
The declaration is the E03 file the Department of Labour asks for. It lists everyone you paid, what they earned, what was subject to UIF and what was contributed, under your employer details.
It is due by the 7th
The card is dated the 1st to the 7th of the following month, which is the Department of Labour's deadline, the same one as the EMP201.
The date does not gate the card. A card showing blocked is waiting for the step before it, and Submit UIF Declaration waits on EMP201 Payment, so the whole SARS chain has to be done first.
What it needs from you
Your Labour UIF number has to be there, and it has to be the real one. It is not your SARS UIF reference. It is the eight digit number the Department of Labour issued you, and E·BIZI Pay writes it as seven digits, a slash, then the last digit. The last digit is a check digit, so a number with a typo in it is not a number E·BIZI Pay can use. It lives on your company profile, alongside the SARS references. Every employee on the run needs an identifying number too: an ID number, a passport number or an alternate number. Someone with none of the three cannot be declared.
Get both of those right before you reach this step, the way you would for the EMP201.
Anyone who left is still on it
The month someone leaves is a month they were paid, so they belong on that month's declaration with the reason they left and their last day.
That is one of the reasons the reason matters when you remove someone. See end an employee's employment.
Doing it again
Completed, Submit UIF Declaration offers Re-execute, which builds and sends the declaration again. Use it if the first one went out with something wrong on it that you have since fixed.
The certificate of service is a different thing
The declaration is monthly and covers everyone. A certificate of service is per person, once, when they leave, and it is what they take to the Department of Labour to claim.
You get it from the employee, not from Processing. Open them under Archived employees and choose Download certificate or Send certificate. See end an employee's employment.
E·BIZI Pay fills it in from their record and your company profile: their position, pay, start date, last day and the reason they left, over your company's UIF and PAYE reference numbers and address. For what the law expects on it, see final pay.
